If you don't know the exact cause, then I hope you understand that I can't speculate without a physical examination to diagnose what it is.       I understand the confusion. One is a line of old filler running from the bottom of the eye onto the cheek area and the other is on the under eye area that appears on and off when I smile I’m wondering if the old filler which has been dissolved twice now is pushing upwards into the under eye. It’s not always apparent

      Hi @DikshaKaushik-bd7sw, Facial asymmetry is natural and normal, and everyone has it, including actors and models. Natural facial asymmetry is most often caused by bone structure, since the two halves of the face are formed separately from each other. Differences in buccal fat from one side of the face compared to the other are also normal. Natural facial asymmetry is usually unnoticeable by others.     I am an older woman, and I am seeing a lot of older women with lip filler. It looks horrible!! Of course, those could be the ones that weren't done well. I think some of the older women in this video look good, but I'm not sure about all of them. I think maybe when lips get really thin, that's when it just doesn't look right with filler. Of course, we I couldn't see their whole face. Some do look really good.

      Hi @samdung5630, The important aspects of lip filler treatment are looking natural; looking proportionate in both the lips, and in context with the whole face. It is safe to be conservative with the amount of filler used initially, as more can be added later on if initial results are undesirable. Remember that it is better for you and the doctor to be conservative at first, and add more later if needed, rather than to add too much at once, then have to dissolve it and start again. As I don't normally treat hypothyroidism directly, but rather treat the effects of inflammation on the eyelids, I suggest you continue with treatment with your endocrinologist for the inflammation. Skin quality improvement can be seen in the succeeding weeks after treatment as the PRP works from within the skin. Improvement to skin quality can also be built upon with subsequent treatments.     Hi doc. I have old HA filler migration under the eye on top of my cheek. Only one eye. Looks like a lump line. My doc has tried to dissolve it twice now and it’s still there!     sir i lost my left eye due to chemical injury and in january 2024 thi evisceration is done without implant due to the infection present in eye, after a month a ball is implanted currently i got my artificial eye but its outcomes are not so good , eye lid of that eye is not opening and closing fully and their is also a sortning of surface in upper fornix side due to which the size of prosthetic eye is also little much smaller than the the right eye (which is fully good) what should i do now pls assist

You do need to see an oculoplastic/ oculofacial plastic surgeon like myself for a proper physical examination, plus an examination of the medical records of your previous procedures with your eyes.     Dr , I am having pitosis it was during my class 10 that I acquired it. I kinda feel insecure whenever someone takes my pic as my left eye comes smaller in pics and I kinda myself feel that my left eyelid muscle is weaker, while I watch at different objects my right eye is stronger and fast but left is kinda back and I feel heavy to lift up my eye pls suggest me any ways should I have to undergo any surgery?? or does it have any other solutions..

      Hi @CricAnshu30, Sorry to hear about your problem. A ptosis exam by an oculofacial/ oculoplastic surgeon, or an ophthalmologist if one is not available, is needed to confirm if you have ptosis. The appropriate procedure is recommended based on the exam. Surgical correction is the only way to treat ptosis.     Hii my hair is really thin and you can see my that my hair line is a bit big and it goes till the back of my head. Growing up i had really thick and long hair but since i turned 17 it started thinning.The thing is that my mom my sister and my brother we all have really thin hair. I’ve been trying hair oiling for a while but I don’t see a big difference. What could be the problem? Is it only genetics? And if so what do you think is the best solution for my hair?

Hair growing back thinner is the problem, which is why a proper diagnosis of the cause of hair thinning is important.       Hi @singhatiit, We don't currently offer the treatment in Texas, but we do have many patients coming to New York or Virginia from around the country and around the world for the TrichoStem Hair Regeneration treatment. Since it is a non-surgical treatment with little to no recovery time, and in many cases no additional treatment, you don't need an extended stay in New York or Virginia. Many patients actually get back on the plane or train, or drive back home the same day as treatment, but to be sure you could do this for a flight, I would assess your general health condition first.       Hi @Random-xd7yo, The outcome of ptosis correction surgery depend on the surgeon performing it, so results can't be generalized based on the results of one person you know. 1. my patients can close their eyes during sleep after surgery 2. ptosis surgery done with insurance is only for eyelid function. I don't accept insurance in my practice, and I perform blepharoplasty (cosmetic eyelid surgery) with ptosis correction when needed 3. your eyelids blink thousands of times a day, every day, so to expect permanent correction is unreasonable 4. whether you are a candidate for ptosis surgery, and the type of procedure needed are determined with a physical examination. Cost is also determined after a physical examination. I hope this clarifies things for you.

    Hey Kevin, I really want your help Been using minoxodil for a while now Like over two years (Been on fin that time too) Now i have been using topical solution form since its a lot cheaper , through all that usage time i did experience some mild inflammation every now and then (not commonly tho) I have read that minoxodil on the long term could cause inflammatory reaction (due to scalp ph levels) So can this be a cause of scarring since scarring could happen due to inflammation (inflammation isn’t severe but have happened quite a bit of times ) I have noticed thinning in these areas too.

      Hi @Blosomawesome, Liquid minoxidil contains propylene glycol to break down the skin barrier for better absorption, but it can also be a skin irritant in many users. Minoxidil doesn't actually thicken thinning hair, but rather delays the natural hair shedding cycle so the scalp looks more full, but if you stop using you are likely to have a sudden and severe hair shed, and lose your gains.       Hi @Cendeyu, There are some studies that say minoxidil affects newly synthesized collagen, and others say they are inconclusive. The specifics about minoxidil side effects are not as relevant in my practice as I generally don't treat beard hair growth as I specialize in treating genetic pattern hair loss, and our TrichoStem Hair Regeneration patients can stop using minoxidil after our injection treatment.       Hi @chenjr1410, Puffy under eyes only when smiling can be due to the contraction of the muscle behind the lower eyelid called the orbicularis oculi, or due to skin bunching in the lower eyelids.     Hello Dr , I had my HT on 15/7/24 and on 27/7/14 I had my head wash with gentle massage with my fingers tips to remove the scabs .i was a able to remove some of the scabs but noticed some hairs in the scabs attached which were removed. I saw lot hairs with the scabs removed while head wash..Is there something to worry or its normal?

      Hi @AGHORI_, Grafted hair follicles should be firmly in place by now, so even if the hair shaft sheds, the grafted follicles can still grow hair again. Any loose grafts by this time didn't take to the scalp, and wouldn't have grown hair anyway. If you are worried about your results, you can follow up with your surgeon who knows details of your transplant surgery, and can physically examine your scalp for any issues. I hope this helps you.

      Hi @tamaka8364, Since minoxidil appears to delay normal hair shedding so you have more hair on your head, if you stop using it you will likely have a sudden hair shed and lose some density. Finasteride works differently by slowing hair thinning in men, but it's a prescription medication so it should only be used with a prescription from your doctor.     Doctor, filler results last like a few months and they migrate easily, isn’t fat grafting better if you don’t have much fat around the eye? Especially if done by a very experienced doctor with decades of experience, and does lump only appear when they put too much in at once and not little cc at a time? I heard only 50% of the fat survives so they usually overfill? Is there many cases of good results or are you saying this because majority of the results are failures?

      Hi @trentlaststand4574, As explained in the video, this is why I don't recommend fat grafting under the eyes: - the shape and texture of grafted fat can be irregular and lumpy once removed from its original blood supply - thin lower eyelid skin (0.5mm thick) can show the lumpy texture of grafted fat - as an eyelid specialist, I often surgically remove grafted fat under the eyes because the patient experienced the above. Doctors who place grafted fat under the eyes often can't remove it if complications occur. Hyaluornic acid filler under the eye is more predictable as it doesn't require a blood supply, the shape and texture are customizable, and if migration does occur, it doesn't migrate far from the original position. Hyaluronic acid filler can be dissolved if needed, while grafted fat requires complex surgery to be removed when complications occur. Incidence of complications are much higher with grafted fat in the lower eyelids than with hyaluronic acid filler. I hope this clarifies things for you.

      @@AmiyaPrasadMD i went to consult plastic surgeon today and he does not recommend dissolving filler as it could potentially destroy the skin, I was gonna dissolve it to get fat grafting. Why does filler always migrate around the temple or near the tear trough but not the cheeks? This is the only reason why I considered fat grafting. I never had trouble with fillers near the cheeks it stays and doesn’t really migrate. My thin lower eyelid is actually very thin genetically i can see the blood vessel, is there a way to thicken skin here without grafting?

    I have a had all this fillers, fractional co2, chemical peel but NOTHING has helped at all and I look at 40 worse than people with 70. Surgery seems the last option. I had NO difference at all

Complications from lower eyelid skin removal are very common, so I only recommend surgery in the rare cases that eyelid skin is folding over itself. Non-surgical skin tightening treatments such as lasers usually require more than one treatment session to avoid overexposure, and overheating.       Hi @lashannawilliams5707, It's not clear if the filler is still there. PRF/EZ Gel only provides short-term volume. If the volume from filler is still there, it would be better to have regular PRP instead to improve skin quality, and skin thickness, without having the appearance of too much volume.       Hi @NirmalaKatageri, Facial asymmetry is natural and normal, and everyone has it, including actors and models. Natural facial asymmetry is most often caused by bone structure, since the two halves of the face are formed separately from each other. Natural facial asymmetry is usually unnoticeable by others. I only advise treatment for facial volume changes due to aging, or if there is a genetic volume deficiency. She told me she makes incisions from the outer corners of the eyes. I can't understand how she can access the fat pads from the outer corners of the eyes. It is farther away from the under eyes.       Hi@@LenGreenblat, The transconjunctival blepharoplasty technique is my preferred approach to treating eye bags, where I leave a little fat behind to prevent a hollow appearance, and maintain a continuous contour between the lower eyelid, and the upper cheek, with no external incision, and no scar.     Hello, I’ve been facing excessive hairfall since last 3 years, an hour after taking second shot of covaxin. What minoxidil appears to do is delay the natural hair shedding cycle, and extend the growth cycle so you have more hair on your scalp, but this effect can be lost with time, or if you stop using it.       Hi @numberzzzzz1, PRP and PRF are lasting treatments because they improve the quality, texture, and thickness of eyelid skin, but they don't actually have a lasting effect on volume., as explained in the video. Collagen-induction does have a limited effect on facial volume in general. Sculptra creates volume with a biological reaction, so it is less predictable than fillers which are far more precise, and customizable. I hope this clarifies things for you.

      Hi @jacobelavanal5880, Your face will still be growing and maturing until your early 20s, so there is still time for issues like these to resolve on their own while your face continues to grow. Dermastamping generally does not create volume under the skin, and I only recommend these treatments are done by professionals trained to do it. At-home microneedling is most often done too aggressively, and without adequate time between sessions for skin to heal. I hope this clarifies things for you.

    Hi docter, my 2 month old son has ptosis on his right eye. Its a monolid eye. How do you differentiate monolid eye from a ptosis eye. Now i am confused on my son's diagnosis. He is undergoing frontalis sling procedure soon. Am i taking a right decision to send him for surgery? My surgeon said he has moderate ptosis as his eyelid covers half of the pupil. Can it be just monolid eye?

      Hi @sojitabraham4414, Sorry to hear about your son's problem. Eyelid ptosis is diagnosed by measuring the function of the muscle that lifts the eyelid called the levator muscle. Monolids are caused by a weak to no connection between the levator muscle, and the eyelid skin, so there is no eyelid crease. Monolids are more of a cosmetic issue, while eyelid ptosis is cosmetic, functional, and also important for vision. Without a physical examination, I would have to defer to your surgeon's judgment. It is important to correct eyelid ptosis in children before age 6 for proper visual development. I hope this clarifies things for you.
